Carolina Medical Homes, LLC to Begin Providing Services on March 1, 2011COLUMBIA, SC – Carolina Medical Homes, LLC announced today that it will enter the South Carolina Medicaid program on March 1st and begin providing coordinated care services through a network of primary care physicians. The contract between Carolina Medical Homes, LLC, a new Medicaid Medical Homes program, and the South Carolina Department of Health and Human Services (SCDHHS) was signed this week.

Carolina Medical Homes (CMH) is a primary care case management program that is contracted with the South Carolina Department of Health and Human Services (SCDHHS) to provide medical homes network services, also known as Care Coordination Services, for Medicaid beneficiaries.
Carolina Medical Homes works with its primary care physicians to support care of both their individual patients and their CMH enrolled "population" of patients. For example, CMH does "welcome calls" for new members, encouraging them to establish with their primary care physician; and uses its data warehouse and analytics to identify and bring to primary care attention those enrolled beneficiaries who may need extra assistance. This includes those with diseases for which primary care physician approved care plans would be helpful (e.g., diabetes, asthma, heart disease or complex cases). CMH also reaches out to those who might benefit from further education. Those using the ER for primary care are encouraged to see their primary care physician, and those who have not seen their primary care physician at all are encouraged to do so. And CMH works with providers to coordinate the care of those discharged from the hospital in a way that improves care and reduces the risk of re-admission.
